Woman Who Came Out As A Trans Man Now De-Transitioning Back To Female
Hopes to help others who are unsure about transitioning.
When Issa Ismail was 19, she announced she was transgender and began transitioning to a male, taking testosterone. Since then, the barber from Detroit, Michigan, has taken hormones for six years, which significantly helped create masculine characteristics, including a deeper voice, facial hair growth, and muscle growth.
Issa, from Michigan, has decided to detransition after experiencing feelings of depression.
She had announced she was transgender at 19 and began transitioning to a male.
The procedure equally helped redistribute fat away from her hips and breasts, changing her total appearance. In 2016, Issa underwent top surgery to remove her breasts, but she began struggling with depression five years later and realized she wasn’t happy living as a man. So, in February 2021, she decided to de-transition back to her female gender and altogether stopped the testosterone therapy.
She took hormones for six years, and they completely changed her body.
The procedure equally helped redistribute fat away from her hips and breasts, changing her total appearance.
Issa has taken to the internet to share her experience, hoping she can help others struggling with their gender and unsure about transitioning. In a recent interview with Real Fix, she recalled: “After the third or fourth-year mark, that was when the feelings of depression were getting bad for me. I kind of had this epiphany – I don’t have to be on hormones for the rest of my life.”
Issa also surgically removed her breasts, but soon realized she wasn’t happy as a man.
In February 2021, she decided to ‘de-transition’ back into a female and stopped the testosterone therapy.
“It was like a shock to me, but the next feeling that I had was a feeling of relief. That’s when I knew I had to listen to myself. I definitely didn’t think that I was going to be this feminine person you see before you. When you see photos of me six years on hormones, I look like the stereotypical cisgender male. My facial hair, my body – how it changed – all of that. Testosterone is such a powerful hormone.”
So far, Issa has taken to the internet to help others who are unsure about transitioning.

Issa was a tomboy up until she was 12 and during her teen years became hyper-feminine.
Issa continued, it’s quite interesting thinking about her life like that and how it goes through stages, adding that the fluidity is so there that she can’t deny it. She reminded others who are struggling to follow their nutrition and heart, saying: “The best advice I can give about transitioning or de-transitioning is listening to yourself and following your intuition, heart, gut feeling.”
“When you see photos of me at six years on hormones, I look like the stereotypical cisgender male,” Issa said.
She added: “My facial hair, my body – how it changed – all of that. Testosterone is such a powerful hormone.”
“The truly is going to give you all the signs that you need. Whatever you start to resonate towards, ebb and flow with that. I want to help trans people feel solidified in their identity, and I wasn’t to also help trans people to feel comfortable and OK with questioning their identity. I really wanted to help both sides. Just because hormones weren’t the life-long path for me doesn’t mean it’s not for the next person.”
Issa felt instant relief when she decided to stop the testosterone, and knew she had to listen to herself.
“The best advice I can give about de-transitioning is truly listening to yourself and following your intuition, heart, gut feeling.”
According to Medical News Today, females who are transitioning to males regularly undergo testosterone therapy or T therapy, reducing the amount of estrogen produced in their bodies. The procedure induces typically masculine physical traits, including male-pattern hair growth and a deepening voice while suppressing the typical female ones and stopping the menstrual flow.
