The incident took place in the testing facility where crew members were checking on the props to make sure everything is ready for the stunts. This is where they work out how the stunts are executed and filmed. After that, they implement them into the production. One of them involved a flipping car and unfortunately, a piece of shrapnel flew and injured Appleby in the head.

Authorities of Toronto announced his death en route to the hospital. Warner Bros. Television released a statement after the tragic death of one of the crew member was confirmed:

“We are heartbroken and devastated by the passing of our treasured colleague, special effects coordinator Warren Appleby, after an accident which occurred at a special effects facility during the preparation and testing for an upcoming shoot. Warren is beloved by all who worked with him during an impressive 25-year career in television and motion pictures.

“The executive producers, along with everyone, in the Titans family, Warner Bros. Television Group and DC UNIVERSE, wish to express our deepest condolences, and heartfelt love and support, to Warren’s family and friends at this most difficult time.”

Due to this sudden issue, production of the show has been put on halt for at least 2 days. Canada’s Ministry of Labor is investigating the death of the crew member.

He always gave you his 100% percent. He was very near and very dear and an integral part of our core team. He will always be beloved and admired.”

He has a 25-year of history in his career, sharing in the credit for David Cronenberg’s ‘Crash’, ‘Hemlock Grove’ and ‘Transporter: The Series.’
